Teriparatide has brought a breakthrough in the cure of osteoporosis. The drug, which is very similar to the natural hormone, parathyroid, works by increasing bone mass and strength, thereby reducing its fragility. The drug greatly enhances the osteoblastic activity then the osteoclastic activity. Thus the once daily schedule of the injection will result in increasing bone strength and mass reducing the fractures. It has also been approved for the treatment of bone resorption in post menopausal women.


The product is used to treat severe osteoporosis. The drug is manufactured by the recombinant DNA technology which provides it the similarity to the body's own parathyroid hormone. It increases the osteoblastic activity of the body thereby reducing bone fragility. The drug has also been used in treating increased fractures in post menopausal women.

Side effects

The use of the product may follow with several side effects like nausea, vomiting, joint pain, runny or stuffy nose, weakness, dizziness, inflammation or irritation of the stomach and cough. Other complications may include sore throat, constipation, diarrhoea, insomnia and anxiety.


The product should not be used in patients with Paget's disease, have high alkaline phosphate level, have had radiation therapy or in children with growing bone as it has high chances of causing bone cancer. It should not be used in patients with severe liver and kidney impairments.


The drug should be stored under refrigeration at 2 to 8 degree Celsius at all times. The delivery device should be recapped every time after the use in order to protect the cartridge from physical damage, heat and light. The dose should be immediately delivered after taking out of the refrigerator. The product should not freeze and should never be used when frozen.


The Forteo is supplied as a clear, isotonic solution in a glass cartridge which is pre assembled in a disposable delivery device for subcutaneous injection. Each pen is filled with 3.3 mL which deliver 3 ml. Each mL contains 250 mcg Teriparatide (corrected for acetate, chloride, and water content), 0.41 mg glacial acetic acid, 0.1 mg sodium acetate (anhydrous), 45.4 mg mannitol, 3 mg Metacresol, and Water for Injection. In addition, hydrochloric acid solution 10% and sodium hydroxide solution 10% may have been added to adjust the product to pH 4.

Important Precautions

The drug should be cautiously used in persons suffering from high blood calcium, unexplained high blood potassium level, undergone radiation therapy, past history of bone cancer or having allergic reactions. Breast feeding and lactating females should consult the doctor before the use of the drug.

Dosage and administration

The recommended dosage for post menopausal women at high risk of fracture is 20 mcg subcutaneously once daily. For the patients with severe osteoporosis the drug dosage is 20 mcg once daily subcutaneously. The drug should be administered as a subcutaneous injection into the thigh or abdominal wall. It should be administered initially under circumstances in which the patient can sit or lie down if symptoms of orthostatic hypotension occur. Person delivering the drug should receive proper training and instructions. Use of the drug for more than 2 years is not recommended.
