Introduction

Colchicine is also the generic name for the brand Colchicine. This drug is widely used medication to relieve symptoms of acute gout. In this medical condition, inflammation is present due to the high levels of uric acid crystals which causes extreme pain, swelling of affected joints as well as redness. The action of Colchicine is to combat the inflammation and relieve pain.


Use

Colchicine drug is intended to treat acute symptoms of gouty arthritis such as inflammation and joint pains; it is also the drug of choice to treat pseudogout inflammation and other unusual diseases such as amyloidodis and scleroderma.


Side effects

The common side effects of Colchicine include gastrointestinal irritations which are related to dosage. Some of these are diarrhoea, abdominal pain, nausea and vomiting. It also causes body malaise, nerve irritation and hair loss. However in worst cases, this drug can largely damage the bone marrow which can lead to severe anaemia. This makes a person prone to infection due to compromised immune system. Hence, patients who are regularly taking Colchicine are required to undergo regular blood count monitoring.


Contraindication

The drug Colchicine is highly contraindicated to pregnant women because it can have detrimental effects of fatal growth. Moreover, it is totally contraindicated in patients with complex renal failure most especially those who are undergoing dialysis due to the fact that up to 20% of Colchicine is excreted by the kidneys unchanged and cannot be removed even by dialysis.

Composition

Colchicine drug is available in tablet forms which contains either 0.5 or 0.6 mg of Colchicine manufactured for oral administration. Colchicine is a derivative of alkaloid Colchicum autumale or autumn crocus. Furthermore, it can also be found in Gloriosa superb or Glory Lily. It also contains numerous substances such as desacetyl, methylcolchicine, desacetylthiocolchicine, colchioside and trimehylcolchicinic acid.


Important Precautions

The Colchicine drug can cause irritation to the gastric mucosa that is why it is recommended not to take the drug in an empty stomach. Furthermore, the risk of toxicity is high in patients who are also taking cholesterol lowering drugs. Symptoms of toxicity may include progressive weakness, sensorimotor polyneuropathy and elevated creatinine kinase. In advanced cases, dementia may be noted and in worst events, respiratory failure and death may be the end result. That is why Colchicine is advisable not to self medicate and always consult medical experts for dosing requirements.


Dosage and administration

The recommended initial Colchicine dose in acute gout attacks is 0.5 mg up to 1mg which is equivalent t 1-2 tablets. This dose should be followed by 0.5 mg tablet every 2 hours until the symptoms have been relieved. The usual dose required to lighten pain attacks is around 4 to 10 mg. Thus, the maximum dose should not be exceeded to avoid gastric complications. Also, the entire course must not be repeated in 3 days. The drug can also be used for prophylactic treatments and oral dose usually ranges from 0.5 mg in a week or up to 3 times a day. However, dosage adjustment may be needed in individuals with impaired kidney functions. It is said that they could be an increased or decreased intervals according to what has been prescribed. Since this drug could irritate the stomach, it is imperative to take it with food.
