Introduction

Penicillamine belongs to the chelator class of pharmaceuticals. It functions as immunosuppressive widely used for treating rheumatoid arthritis. It is also used globally in the treatment of Wilson's disease and scleroderma. It works by reducing the rheumatoid factor thereby preventing the cross linking of collagen.


Use

Penicillamine is used in treating cystinuria, a hereditary disorder where there is formation of cystine stone. It is used to treat Wilson's disease a genetic disease related with copper deposition. In this disease penicillamine functions as chelating agent and binds to the accumulated copper and eliminates it from the body via urination. It is used as second line of treatment in cases of arsenic poisoning.


Side effects

Nausea, itching, vomiting, swelling of lips, abdominal cramps, rashes, abdominal pain, allergic reactions, weight loss, diarrhea and dysguesia that is the loss of sense of taste are some of the side effects.


Contraindication

In pregnancy, lactation, penicillamine-related aplastic anemia or agranulocytosis and in patients of rheumatoid arthritis who have a past history of renal insufficiency penicillamine is contraindicated.

Keep it out of sight and out of reach of children who may swallow it accidentally. Store it at room temperature in a place that is protected from direct heat sunlight and moisture. Do not use the medicine after the date of expiry.


Composition

Penicillamine is available in form of capsule that contains 250 mg of D-Penicillamine (2S) 2-amino-3-methyl-3-sulfanyl-butanoic acid. Penicillamine is water soluble and has pKa values of 1.83, 8.03, and 10.83. It is fathomable in alcohol.


Important Precautions

If you are pregnant or planning to become pregnant please consult your doctor before taking this drug. Physicians should educate the patients about penicillamine toxicity and patients must immediately report to the doctor in cases of symptoms of penicillamine toxicity. Pemphigus may be a late complication of this drug. It is also associated with good pasture syndrome, aplastic anemia and thrombocytopenia. Proteinuria, obliterative bronchiolitis and hematuria are also associated with the intake of penicillamine.


Dosage and administration

Always consume the medicine with a glass of water on an empty stomach one to two hours before the meals. Carefully read the instructions on the prescription label. It should be taken at least one hour apart from other medicines or milk. Do not skip the doses of medicine. Never stop taking the medicine abruptly even if you feel that you have recovered completely do so only after consulting the doctor.
