Gemfibrozil is used in adult patients (particularly overweight) with diabetes type 2 (non-insulin-dependent). Gemfibrozil can be used as single drug in patients for whom metformin (an antidiabetic drug) is not indicated. Gemfibrozil can be used in combination with metformin ("dual therapy") in patients with type 2 diabetes cannot achieve satisfactory disease control with metformin alone or in combination with a sulphonylurea (another type of antidiabetic drug) when metformin is indicated. Gemfibrozil can be used also in combination with two drugs - metformin and sulphonylurea ('triple therapy'), in patients who cannot achieve adequate control of disease, although using dual oral therapy.

Type 2 diabetes is a disease in which the pancreas does not produce enough insulin to control the concentration of glucose (sugar) in blood or when the body cannot use insulin effectively. Rosiglitazone, the active ingredient in Gemfibrozil, increases insulin sensitivity of cells (fat, muscle and liver), which means that the body makes better use the insulin it produces. As a result, blood glucose concentration is reduced. This facilitates the control of type 2 diabetes. Treatment of type II diabetes with Lopid completes diet and exercise.

Side effects

The most common side effects of treatment with Gemfibrozil (seen in patients 1 to 10 of 100) are anemia (low red blood cells), hypercholesterolemia (elevated cholesterol in the blood), hypertriglyceridemia (elevated concentrations of triglycerides, a type of fat), hyperlipidemia (increased concentration of lipids, another type of fat), weight gain, increased appetite, cardiac ischemia (reduced oxygen to the heart muscle), constipation, bone fractures (broken bones), hypoglycemia (glucose concentration reduction blood), and edema (swelling).

Gemfibrozil should not be used in people who may be hypersensitive (allergic) to rosiglitazone or any of the other ingredients or patients suffering from heart failure, liver problems, an "acute coronary syndrome" such as unstable angina (a severe chest pain with intensity), certain types of heart attack or complications of diabetes (diabetic ketoacidosis or diabetic pre-coma). Gemfibrozil doses may require modification if given in combination with some drugs such as Gemfibrozil or rifampicin.

Store Gemfibrozil drug away from direct light, heat and humidity.

Gemfibrozil is a medicine containing the active ingredient rosiglitazone. It is available as tablets (pink: 2 mg; orange: red-brown 4 mg or 8 mg).

Important Precautions

There are no necessary precautions to be taken.

Dosage and administration

Gemfibrozil may be taken with or without food. The recommended starting dose is 4 mg daily, administered in a single dose or two doses of 2 mg. This dose may be increased to 8 mg daily after eight weeks if you need a better control of blood sugar concentrations, given either a single dose or two doses of 4 mg. Dose escalation should be but with caution in patients who are receiving, also, sulfonylureas, because of the risk of fluid retention.
