Introduction

Tacrolimus is an immunosuppressive drug that is mainly prescribed to the patient undergoing allogeneic organ transplantation operation. As an organ is transplanted from another part, there might be possibility to develop the antigen-antibody reaction as the host body considers the transplanted tissue as a foreign body and there might be the possibilities of rejection of the transplantation. Therefore, this drug is prescribed to suppress immune system of the host body to reduce the chances of antibody-antigen reaction. This is used as an ointment in the site of allogeneic transplantation. The prognosis is said to be good after the up taking of the transplantation after the use of this ointment.


Use

This ointment is an immunosuppressive agent generally prescribed to prevent organ rejection during allogenic transplantation. This ointment decreases the activity of the immune system to prevent the transplanted organ from rejection.


Side effects

As it is applied to the skin topically, it produces the side effects in the skin. The chief side effects of this ointment are photosensitivity reaction, skin discoloration, skin ulcers, skin overgrowth, excessive sweating and white patches over the skin. Some other adverse effects are joint inflammation and nail disorders.


Contraindication

The chief contraindication of applying this ointment is the hypersensitivity reaction. No other significant contraindications are found being associated with the use of this ointment. Only care should be taken to not let the ointment go into the eyes or the mouth.


storage

The pack of the topgraf ointment is generally kept in the room temperature ranging from 15 degree Celsius to 30 degree Celsius. The ointment pack should be kept away from the reach and sight of the children and the pets. Direct sunlight exposure and the moisture exposure should be avoided while storing the tube of the ointment.


Composition

This ointment is composed of tacrolimus as the pharmacological agent in the strength of 0.1% w/w as monohydrate.


Important Precautions

Before prescribing the ointment the clinician should be conscious about the allergic history of the patient. Cautions should be taken with the cardiac, renal or hepatic diseases; those are in taking other drugs. Special caution is taken in the elderly patients, children, and breast feeding children also. The patient should be avoided the exposure to the sunlight as it may precipitate carcinogenic process in skin. This drug may increase blood pressure, so the blood pressure should be checked. Vaccinations should be avoided during the use of the chemical. Excess dose should be avoided.


Dosage and administration

This ointment with 0.03% or 0.1% 10gm is applied topically over the area is transplanted with the allogenic tissue. A thin layer of this ointment is applied over the affected skin twice in a day.
