Introduction

Acyclovir belongs to the group of antiviral agents which is also one of the most commonly prescribed drugs against a variety of viral infections such as the CMV, HSV, etc. The drug is given to the children usually in the form of syrup and can be given in case of viral infections. The drug belongs to the group of synthetic nucleoside analogue and is active against herpes viruses mostly.


Use

The primary use of acyclovir is to have the patients free from virus infections, mostly of the herpes simplex family. They also act on other viruses also, but the HSV is most important and effective. Primary or recurrent herpes simplex infections are prescribed with these drugs. CNS infections by herpes simplex viruses are also given this particular syrup. Genital herpes can be treated by oral administration of acyclovir. Varicella zoster and herpes simplex in immunocompromised patients can be treated with the drug.


Side effects

Nausea, vomiting, diarrhea, and other GI symptoms are common in the patients taking acyclovir syrup. Rashes and haematological symptoms such as anemia might be seen in some cases which is because of the hypersensitivity reactions. Rise in liver enzymes, keratopathy, blepharitis, conjunctivitis, erythema, etc are found rarely in few patients. Besides these few effects, the overall performance is the oral syrup form is not very dangerous.


Contraindication

Hypersensitivity of the drug to the patients means that the drug cannot be given, even if there was only a few episodes some times back. Pregnancy and breast feeding mothers should not be prescribed with the drug as it will cause harm to the fetus or suckling infant.


storage

The syrup should be kept away from the reach of children and infants. It should be kept at room temperatures ranging from 15-30 deg C and it should not be allowed to be frozen.


Composition

The drug contains the acyclovir drug of 400 mg in solution of 5 ml. Along with that there are other inactive agents that act as preservatives or vehicles for the syrup.


Important Precautions

In case of pregnancy and breast feeding, the doctor should be informed so that the drug continuation and withdrawal can be decided. Renal damage and impairment requires the drug to be stopped as the condition can get aggravated. Neurological symptoms are seen in many cases where hypoxia can aggravate the symptoms and therefore the drug has to be taken with care and precautions. Serious hepatic and electrolyte abnormalities require the drug to be stopped or else adequate hydration can be maintained and the drug effect is to be countered.


Dosage and administration

The syrup is given in different doses for patients with difference in the severity of infection. The usual dose is around 5 mg/kg body weight at 8 hourly intervals for continuous 5-7 days. For simpler infections, the dose can be reduced to 3 mg/kg but the frequency per day cannot be lowered. It is to be taken orally without any influence of food.
